Evaluation Questions and Checklist
- Please include a brief description of your NGO - history/management team/achievements.
- Where is the land for the Phoenix Center located?
- What size land will the Center be placed on?
- What are the conditions of the road access to the land?
- How many people will be using the facility? Number of Adults? Youth? Elders?
- Is there water available? If so, what kind? If not, where is the nearest water source? How deep does the well have to be?
- Is there an energy source?
- How much rain do you get per year? If a rainy season, how long does it last?
- Is food needed in the community?
- What foods are available now? What is the common food that is eaten? How is it cooked?
- Do community members grow their own food?
- Is there fertile land for farming?
- What are the main crops being farmed in your area?
- Is education needed in the community?
- What is the education structure the students have now?
- For education, what are the ages of the students?
- Does the community speak English? What language is spoken?
- How many in the community are familiar with computers?
- Do you work with other local/regional/national/global NGO's?
- Is housing needed? If so, how many homes? How many people per home?
- How many adults are able to assist the building contractors for basic tasks?
- Will you need security? What are the potential security risks for the Center?
- What is the population of the communities near the Center?
- What are the health issues in the region?
- What health facilities are available now? How many local doctors/nurses are in the Phoenix Center region?
- What recreational sport is preferred? Football? Cricket? Tennis? Wrestling? Basketball? Golf? (you can choose more then one)
- Please list some of the issues that you feel should be addressed for the community.
- Please add a brief description of a viable business idea for your Center. You may include more then one business idea.
- Please note the list of skills that should be identified within the Phoenix Centers community. Make a list of the areas outside support will be required.
